I found all pages of diary but quest won't end... Is it bug or i missed something??
In a pond close to the exit of the dungeon you should find the body of the diary's author. Clicking on him lets you finish the quest.
However, the apothecarius sometimes doesn't spawn in modded games. I don't know which mods exactly cause the problem, but if he really isn't where he is supposed to be, try to deactivate them, if possible.

Does the whole quest happen at the cave you find the first book? Or do you search around the forest?
