I guess I am not sure what you are asking. What are you confused about precisely? If you have finished the main quest then great, I do not think that affects the side quests and other places to visit, so, I would not start a new game necessarily. If you feel as though you have missed out on things you could always load a save previous to the main quest endgame and search the world from there, still having the end of the main quest to look forward to. Why don't you give some specifics as to what you are worried about missing? Put them in a spoiler if you are worried about giving something away.
I am going to list some things to do that you may not have done. They aren't spoilers really, just general suggestions, but I am going to hide them regardless:

Hi.
Totally agree with Blink.
But .. If its sidequests and dungeons etc you think you might have missed, then being the level you are after the MQ, going back to some earlier areas is going to be soooooo easy.
The suggestion to use a savegame just before end of MQ is a good one.
You could always wait to start afresh with a new character when a new update/s come arround.
p.s I think I've missed one of those areas in your spoiler!!!!!

thanks well
after enjoying the main quest you can clearly see some developer talent there!
i will start over a new game to roam around the world cause its so relaxing
great for loosing some stress
i dont know about you guys but it looks so fine on my system
you should see this on a flatscreen 1680*1050!
so will play it some more specially the landscape and the major city i think are the greatest assets of this game
<-- does this bring back memories?

<-- does this bring back memories? Hi.
Believe it or not, I was just considering firing the GOTY version up (never ever completed it), it was and still is in my top three rpg greats of all time.
But I've also just downloaded Myar Aranth and Arktwend. So while I have created a new character for Nehrim, do not want to go too far into the game until I see what next update might include. Also teaching myself moding and putting some diff armour/weapons arround Nehrim in difficult to get to places.
p.s Blink - what is in that second place you have put in your spoiler list. Like to know if I have been to it!!
